About the Foundation
The Foundation is a not-for-profit 501c3 organization with the purpose of developing specialty disease management initiatives and cost management programs for individuals with chronic, complex, and complicated conditions. Initially, programs were developed for high-risk patients and funded through grants from the Centers for Medicare and Medicaid Services (CMS).
The Foundation is currently focused on working with self-funded employers, provider organizations, research organizations, and payors to manage the care of individuals who can benefit from Foundation programs and to research alternative methods of healthcare approaches. We design, develop, implement, and operate programs that provide focused and efficient care for high-cost individual. Additionally, we collaborate with well-respected providers and healthcare networks to implement best outcomes and eliminate unnecessary costs to enhance patient outcomes for all of our programs. All programs result in quality study outcomes.

Mission & Vision
The mission of the Foundation is to affect the 5% of the population that represents 57% of the cost of healthcare delivery. It is the vision of the Foundation to target the highest cost and most complex health conditions, and assist in developing programs, protocols, and approaches that are both more affordable and effective. The concept of better healthcare and better outcomes is achieved through the delivery of quality healthcare at the right place and right time. Included in innumerable studies of the U.S. healthcare system is the finding that the extraordinarily high cost all to often does not assure high quality outcomes. The Foundation will design, develop and implement better ways to care for the most expensive diseases and conditions through its own programs and in collaboration with partner organizations.
